Materialele prezentate sunt esen\u0163a parcurgerii eviden\u0163elor \u015ftiin\u0163ifice care vin s\u0103 sus\u0163in\u0103 conceptul de <strong>fragilitate a v\u00e2rstnicului <\/strong>dar \u015fi aspecte locale din t\u0103rile europene care vizeaz\u0103 aceast\u0103 tem\u0103.<\/p>\n<p>Coordonatorul general al proiectului dl. Lecadio Rodriguez-Man\u00e3s \u00eempreun\u0103 cu coordonatorul adjunct, dna In\u00e9s Garcia-S\u00e1nchez au prezentat documentul concluziv reie\u0219it dup\u0103 2 ani de conlucrare fructuoas\u0103 a reprezentan\u0163ilor autorit\u0103\u0163ilor centrale, regionale \u0219i a partenerilor, din statele participante la program. Raportul \u2013 ABORDAREA PREVENTIV\u0102 A FRAGILIT\u0102\u0162II, cuprinde, pe l\u00e2ng\u0103 un ghid despre <strong>fragilitatea v\u00e2rstnicului <\/strong>\u015fi recomand\u0103ri pentru dezvoltarea de politici de s\u0103n\u0103tate care s\u0103 sus\u0163in\u0103 acest concept.<\/p>\n<p><a href=\"https:\/\/anmcs.gov.ro\/ro\/wp-content\/uploads\/2019\/01\/madrid-4.jpg\" data-rel=\"lightbox-image-3\" data-rl_title=\"\" data-rl_caption=\"\" title=\"\"><img loading=\"lazy\" decoding=\"async\" class=\"size-medium wp-image-6070 alignright\" src=\"https:\/\/anmcs.gov.ro\/ro\/wp-content\/uploads\/2019\/01\/madrid-4-300x168.jpg\" alt=\"\" width=\"300\" height=\"168\" srcset=\"https:\/\/anmcs.gov.ro\/ro\/wp-content\/uploads\/2019\/01\/madrid-4-300x168.jpg 300w, https:\/\/anmcs.gov.ro\/ro\/wp-content\/uploads\/2019\/01\/madrid-4-768x431.jpg 768w, https:\/\/anmcs.gov.ro\/ro\/wp-content\/uploads\/2019\/01\/madrid-4.jpg 800w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\" \/><\/a>Cre\u0219terea num\u0103rului popula\u021biei \u00een v\u00e2rst\u0103 este una dintre problemele majore actuale \u0219i mai ales de viitor ale Europei.<\/p>\n<p>Predic\u021biile, bazate pe Raportul European al \u00cemb\u0103trinirii din 2015 arat\u0103 c\u0103 num\u0103rul persoanelor peste 65 de ani va cre\u0219te de la 18% la 28% p\u00e2n\u0103 \u00een 2060 iar a celor peste 80 de ani, cel pu\u021bin se va dubla (de la 5% la 12%). \u00cens\u0103 cel pu\u021bin legat de subiectul <strong>fragilit\u0103\u021bii v\u00e2rstnicului<\/strong> nu propor\u021bia conteaz\u0103 ci mai ales num\u0103rul \u00een continu\u0103 cre\u0219tere de persoane \u00een v\u00e2rst\u0103 care au riscul major de a deveni <strong>fragili<\/strong> \u0219i care foarte probabil vor dezvolta dizabilit\u0103\u021bi.<\/p>\n<p>Procesul de \u00eemb\u0103tr\u00e2nire \u00een sine cre\u015fte riscul de <strong>fragilitate<\/strong> dar nu genereaz\u0103 automat <strong>fragilitatea<\/strong>, care este rezultatul interac\u0163iunii mai multor factori, precum procesul de \u00eemb\u0103tr\u00e2nire, bolile cronice, stilul de via\u0163\u0103, mediul \u015fi elementele cultural-religioase etc.<\/p>\n<p>\u00cen ciuda numeroaselor discu\u021biilor legate de o defini\u021bie de comun acord agreat\u0103, este larg acceptat c\u0103 termenul de <strong>frailty &#8211;<\/strong> <strong>fragilitatea v\u00e2rstnicului<\/strong> este indubitabil legat de v\u00e2rsta persoanelor; persoana t\u00e2n\u0103r\u0103 poate avea un status de <strong>vulnerabilate<\/strong> dar nu de <strong>fragilitate<\/strong>. <strong>Fragilitatea v\u00e2rstnicului<\/strong> se manifest\u0103 printr-o scadere fizilogic\u0103 a resurselor biologice \u0219i o cre\u0219tere a vulnerabilit\u0103\u021bii, cu posibile consecin\u021be precum dizabilit\u0103\u021bile, institu\u021bionalizarea, spitalizarea sau decesul.<\/p>\n<p><em>\u00a0<\/em><em>\u201dFragilitatea v\u00e2rstnicului este o sc\u0103dere progresiv\u0103 a func\u021bionalit\u0103\u021bii sistemelui fiziologic, \u00een func\u021bie de v\u00e2rst\u0103, care duce la sc\u0103derea capacit\u0103\u021bii intrinseci, a rezervelor func\u021bionale, conferind astfel o vulnerabilitate extrem\u0103 fa\u021b\u0103 de factorii de stres \u0219i o cre\u0219tere a riscului fa\u021b\u0103 de elementele cu efect negativ asupra s\u0103n\u0103t\u0103\u021bii\u201d (OMS, 2015).<\/em><\/p>\n<p>Cu toate acestea, <strong>fragilitatea v\u00e2rstnicului <\/strong>poate fi prevenit\u0103 sau \u00eent\u00e2rziat\u0103 asigur\u00e2nd o vie\u021buire mai lung\u0103 \u0219i cu afect\u0103ri mai reduse sau controlabile ale s\u0103n\u0103t\u0103\u021bii. Astfel, cu m\u0103suri adecvate, luate din timp, de prevenire dar \u0219i de reconfigurare a sistemelor sanitare, s-ar putea controla impactul major previzionat de tendin\u021ba accentuat\u0103 de \u00eemb\u0103trinire a popula\u021biei europene asupra sustenabilit\u0103\u021bii sistemelor de s\u0103n\u0103tate din \u021b\u0103rile membre.<\/p>\n<p>Comisia Uniunii Europene lu\u00e2nd act de aceast\u0103 situa\u021bie, a hot\u0103r\u00e2t \u00eempreun\u0103 cu 33 de parteneri din 22 state membre, s\u0103 demareze \u00eencep\u00e2nd cu 2015, proiectul ADVANTAGE ca o ac\u021biune comun\u0103 finan\u021bat\u0103 prin al Treilea Program de Sanatate al Uniunii Europene 2014-2020.<\/p>\n<p>Proiectul \u015fi-a propus s\u0103 sensibilizeze societatea european\u0103, popula\u0163ia, autorit\u0103\u0163ile, s\u0103 creeze o cultur\u0103 a respectului fa\u0163\u0103 de persoanele <strong>v\u00e2rstnice fragile<\/strong> \u015fi s\u0103 ia m\u0103suri adecvate nu numai de protec\u0163ie \u015fi tratament adecvat ci m\u0103suri, chiar personalizate de prevenire \u015fi protec\u0163ie fa\u0163\u0103 de fenomenul de <strong>fragilizare <\/strong>a v\u00e2rstnicului.<\/p>\n<p>Romania se situeaz\u0103 \u00eentr-o pozi\u021bie considerat\u0103 \u201dechitabil\u0103\u201d \u00een ceeace prive\u0219te abordarea conceptului de <strong>fragilitate a v\u00e2rstnicului<\/strong>, fa\u021b\u0103 de alte \u021b\u0103ri precum Marea Britanie, Spania, Fran\u021ba, Italia sau Finlanda care au o strategie na\u021bional\u0103 \u0219i o experien\u021b\u0103 notabil\u0103 pe aceast\u0103 tem\u0103.<\/p>\n<table>\n<tbody>\n<tr>\n<td width=\"217\">Categoria de \u00eencadrare \u00een func\u021bie de abordarea conceptului de <strong>fragilitate a v\u00e2rstnicului<\/strong><\/td>\n<td width=\"349\">Statele membre ale programului ADVANTAGE<\/td>\n<\/tr>\n<tr>\n<td width=\"217\"><strong>SUSTENABIL\u0102 <\/strong><sup>1<\/sup><\/td>\n<td width=\"349\">&#8211;<\/td>\n<\/tr>\n<tr>\n<td width=\"217\"><strong>AVANSAT\u0102 <\/strong><sup>2<\/sup><\/td>\n<td width=\"349\">Finlanda, Fran\u021ba, Italia, Spania, Marea Britanie<\/td>\n<\/tr>\n<tr>\n<td width=\"217\"><strong>BINE DEZVOLTAT\u0102 <\/strong><sup>3<\/sup><\/td>\n<td width=\"349\">Belgia, Irlanda, Olanda, Polonia<\/td>\n<\/tr>\n<tr>\n<td width=\"217\"><strong>ECHITABIL\u0102 <\/strong><sup>4<\/sup><\/td>\n<td width=\"349\">Austria, Cipru, Germania, Grecia, Malta, Portugalia, Rom\u00e2nia, Slovenia<\/td>\n<\/tr>\n<tr>\n<td width=\"217\"><strong>BAZAL\u0102 <\/strong><sup>5<\/sup><\/td>\n<td width=\"349\">Bulgaria, Croa\u021bia, Ungaria, Lituania<\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<p><em>(1) exist\u0103 o strategia na\u021bional\u0103 evaluat\u0103 pe tema fragilit\u0103\u021bii v\u00e2rstincului sau un plan agreat pentru sus\u021binerea acesteia;<\/em><\/p>\n<p><em>(2) exist\u0103 o strategia na\u021bional\u0103 pe tema fragilit\u0103\u021bii v\u00e2rstincului;<\/em><\/p>\n<p><em>(3) sunt dezvoltate\/func\u021bionale interven\u021bii\/programe relevante pe tema fragilit\u0103\u021bii v\u00e2rstincului;<\/em><\/p>\n<p><em>(4) ceva este f\u0103cut \u00een domeniul fragilit\u0103\u021bii v\u00e2rstincului;<\/em><\/p>\n<p><em>(5) nimic nu este f\u0103cut \u00een domeniul fragilit\u0103\u021bii v\u00e2rstincului.<\/em><\/p>\n<p>Modelele de abordare a persoanelor v\u00e2rstnice nu trebuie s\u0103 fie centrate doar pe tratarea patologiilor cronice pe care le au, ci \u0219i pe elemente de \u00eengrijire \u0219i sprijin pentru a preveni declinul, fragilitatea \u0219i dizabilit\u0103\u021bile. De\u0219i sunt legate \u00eentre ele, totu\u0219i nu exist\u0103 o rela\u021bie linear\u0103 \u00eentre patologiile cronice \u0219i func\u021bionalitatea persoanei, func\u021bionalitatea fiind defapt factorul principal al calit\u0103\u021bii \u0219i satisfac\u021biei vie\u021bii v\u00e2rstnicului.<\/p>\n<p>Fragilitatea se caracterizeaz\u0103 printr-o dinamicitate care se poate modifica \u00een func\u021bie de abilitatea v\u00e2rstnicului de a \u00eendeplinii activit\u0103\u021bi independent. Indiferent de nivelul ei, <strong>fragilitatea v\u00e2rstnicului <\/strong>reprezint\u0103 un grad de risc &#8211; risc de c\u0103dere, risc de efecte adverse la medica\u021bie\/tratament, risc de pierdere a independen\u021bei, risc de agravare a patologiilor sau chiar deces.<\/p>\n<p>De aceea, preven\u021bia cuprinz\u0103toare a <strong>fragilit\u0103\u021bii v\u00e2rstnicului<\/strong> presupune m\u0103suri \u00een domeniul vie\u021bii s\u0103n\u0103toase, educa\u021biei pentru s\u0103n\u0103tate &#8211; efectuarea constant\u0103 de exerci\u021bii fizice (30-45 minute de trei ori pe s\u0103pt\u0103m\u00e2n\u0103), mi\u0219care zilnic\u0103, activit\u0103\u021bi casnice sau de \u00eentre\u021binere personal\u0103, activit\u0103\u021bi de interrela\u021bionare, de socializare pentru a reduce izolarea, depresia, anxietatea \u0219i imobilitatea. S-a eviden\u021biat c\u0103 alimenta\u021bia, o diet\u0103 s\u0103n\u0103toas\u0103 gen dieta mediteranian\u0103, poate contribui, pe l\u00e2ng\u0103 activit\u0103\u021bile enumerate mai sus, la sc\u0103derea riscului de a deveni <strong>v\u00e2rstnic fragil<\/strong>. O mare aten\u021bie trebuie acordat\u0103 polipragmaziei c\u0103ci fragilitatea poate induce efecte secundare importante.<\/p>\n<p><a href=\"https:\/\/anmcs.gov.ro\/ro\/wp-content\/uploads\/2019\/01\/madrid-5.jpg\" data-rel=\"lightbox-image-4\" data-rl_title=\"\" data-rl_caption=\"\" title=\"\"><img loading=\"lazy\" decoding=\"async\" class=\"size-medium wp-image-6072 alignright\" src=\"https:\/\/anmcs.gov.ro\/ro\/wp-content\/uploads\/2019\/01\/madrid-5-300x168.jpg\" alt=\"\" width=\"300\" height=\"168\" srcset=\"https:\/\/anmcs.gov.ro\/ro\/wp-content\/uploads\/2019\/01\/madrid-5-300x168.jpg 300w, https:\/\/anmcs.gov.ro\/ro\/wp-content\/uploads\/2019\/01\/madrid-5-768x431.jpg 768w, https:\/\/anmcs.gov.ro\/ro\/wp-content\/uploads\/2019\/01\/madrid-5.jpg 800w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\" \/><\/a>Dezbaterile \u0219i consensul participan\u021bilor la proiect au dus la formularea urm\u0103toarelor <strong>concluzii generale<\/strong>:<\/p>\n<p>&#8211; preven\u021bia <strong>fragilit\u0103\u021bii v\u00e2rstnicului<\/strong> trebuie s\u0103 devin\u0103 o prioritate a s\u0103n\u0103t\u0103\u021bii publice la nivelul Uniunii Europene;<\/p>\n<p>&#8211; societatea civil\u0103, comunit\u0103\u021bile \u0219i toate p\u0103r\u021bile interesate trebuie activ implicate;<\/p>\n<p>&#8211; politica de s\u0103n\u0103tate trebuie s\u0103 fie centrat\u0103 at\u00e2t pe persoan\u0103 c\u00e2t \u0219i pe popula\u021bie;<\/p>\n<p>&#8211; este necesar un screening sistematic pentru a identifica din timp <strong>fragilitatea v\u00e2rstnicului<\/strong>;<\/p>\n<p>&#8211; pentru optim rezultat, preven\u021bia \u0219i interven\u021biile timpurii trebuie s\u0103 se bazeze pe o <strong>Evaluare Geriatric\u0103 Complet\u0103<\/strong>;<\/p>\n<p>&#8211; serviciile \u0219i suportul necesar trebuie g\u00e2ndite \u0219i oferite integrat \u0219i centrat pe persoan\u0103;<\/p>\n<p>&#8211; preg\u0103tirea personalului medical \u0219i nemedical trebuie s\u0103 fie specific\u0103;<\/p>\n<p>&#8211; trebuie investit \u00een cercetarea \u0219i evaluarea <strong>fragilit\u0103\u021bii v\u00e2rstnicului<\/strong>;<\/p>\n<p>&#8211; trebuie stimulat\u0103 utilizarea tehnologiilor de informatic\u0103 medical\u0103 \u0219i a aplica\u021biilor digitale din domeniu.<\/p>\n<p>Faza de implementare a proiectului presupune eforturi conjugate, pe multiple planuri, at\u00e2t politic, ale autorit\u0103\u0163ilor centrale, c\u00e2t \u015fi cet\u0103\u0163ene\u015fti, comunitare.<\/p>\n<p>Ca atare, pentru o politic\u0103 coerent\u0103 privind <strong>fragilitatea v\u00e2rstnicului<\/strong>, fiecare stat membru UE trebuie s\u0103 parcurg\u0103 urm\u0103torii pa\u0219i:<\/p>\n<p>&#8211; s\u0103-\u0219i identifice la nivel na\u021bional sau, dup\u0103 caz, regional domeniile de ac\u021biune legate de <strong>fragilitatea v\u00e2rstnicului<\/strong> \u0219i s\u0103 le pozi\u021bioneze adecvat \u00een politicile lor sanitare;<\/p>\n<p>&#8211; s\u0103-\u0219i stabileasc\u0103 obiective \u0219i rezultate de atins la nivel na\u021bional sau, dup\u0103 caz, regional, legate de <strong>fragilitatea v\u00e2rstnicului<\/strong>;<\/p>\n<p>&#8211; s\u0103 implementeze interven\u021bii dovedite a fi eficiente pentru a reduce factorii de risc lega\u021bi de <strong>fragilitatea v\u00e2rstnicului<\/strong>;<\/p>\n<p>&#8211; s\u0103 implementeze interven\u021bii dovedite a fi eficiente pentru a gestiona la nivel individual <strong>fragilitatea v\u00e2rstnicului<\/strong>;<\/p>\n<p>&#8211; s\u0103 implementeze interven\u021bii dovedite a fi eficiente pentru preg\u0103tirea specific\u0103 a personalului sanitar \u0219i de suport \u00een domeniul <strong>fragilit\u0103\u021bii v\u00e2rstnicului<\/strong>;<\/p>\n<p>&#8211; s\u0103 monitorizeze progresul \u0219i s\u0103 m\u0103soare rezultatele, \u021bin\u00e2nd cont de indicatorii propu\u0219i;<\/p>\n<p>&#8211; s\u0103 elaboreze ac\u021biuni \u0219i cooper\u0103ri multisectoriale care privesc <strong>fragilitatea v\u00e2rstnicului<\/strong>;<\/p>\n<p>&#8211; s\u0103 promoveze colaborarea interna\u021bional\u0103 \/ interregional\u0103 pentru transferul \/ promovarea exemplelor de bune practici \u00een domeniul <strong>fragilit\u0103\u021bii v\u00e2rstnicului<\/strong>.<\/p>\n<p>\u00cen programul ADVANTAGE partenerii din Rom\u00e2nia sunt:<\/p>\n<p>&#8211; Centrul Na\u021bional de S\u0103n\u0103tate Mintal\u0103 \u0219i Lupt\u0103 Antidrog;<\/p>\n<p>&#8211; \u0218coala Na\u021bional\u0103 de S\u0103n\u0103tate Public\u0103, Management \u0219i Perfec\u021bionare \u00een Domeniul Sanitar Bucure\u0219ti (SNSPMPDSB);<\/p>\n<p>&#8211;
